How to Apply for Death Certificate

Step 1: Gather Required Documents

For New Registration (Within 21 Days):

• Hospital death summary or death notification from hospital
• Hospital death registration form (if provided by hospital)
• Deceased person's Aadhaar card (if available)
• Proof of relationship (family member's ID - Aadhaar, voter ID)
• Proof of address (Aadhaar, ration card, utility bill)
• Applicant's ID proof (person applying for certificate)

For Delayed Registration (After 21 Days):

• All documents mentioned above
• Affidavit from family member explaining the delay
• Witness statements (2 witnesses who can confirm the death)
• Medical records or post-mortem report (if available)
• Additional fees may apply

Step 2: Apply Online

Option 1: e-Sevai Portal

1. Visit e-Sevai portal: https://eservices.tn.gov.in
2. Register or login to your account
3. Select "Death Certificate" from the services list
4. Click "Apply Online"
5. Fill in all required details (deceased person's name, date of death, place of death, cause of death, applicant details)
6. Upload scanned copies of required documents
7. Pay the application fee online
8. Submit the application
9. Note down the application number

Option 2: Local Corporation/Municipality Portal

1. Visit your local corporation/municipality website
2. Look for "Death Certificate" or "Civil Registration" section
3. Register or login
4. Fill the online application form
5. Upload documents
6. Pay fees and submit

Step 3: Offline Application (Alternative)

1. Visit your local corporation/municipality office
2. Collect the death certificate application form
3. Fill the form with all required details
4. Attach photocopies of all required documents
5. Submit the form along with original documents for verification
6. Pay the application fee
7. Get acknowledgment receipt

Step 4: Track Application Status

1. Visit e-Sevai portal or corporation website
2. Go to "Track Application" or "Application Status"
3. Enter your application number
4. View current status
5. You'll receive SMS/email updates on your registered mobile/email

Step 5: Download Certificate

1. Once approved, you'll receive notification
2. Login to e-Sevai portal or corporation website
3. Go to "My Applications" or "Download Certificate"

6. Print if needed
7. You can also collect physical copy from corporation office

Get Duplicate Death Certificate

1. Visit e-Sevai portal or corporation website
2. Select "Duplicate Death Certificate" service
3. Enter details like date of death, place of death, deceased person's name
4. Upload ID proof and address proof
5. Pay duplicate certificate fee (₹50-100)
6. Submit the application
7. Download duplicate certificate once approved
